What is a primary care network (PCN)?
A primary care network consists of groups of general practices working together, and in partnership with community, mental health, social care, pharmacy, hospital and voluntary services in their local area, to offer more personalised, coordinated health and social care to the people living in their area.
There are 42 PCNs in Kent and Medway and the majority of networks serve between 30,000 to 50,000 patients. Networks will be aligned to their regional integrated care partnership - this will support them to work more closely with other health and care partners in the area.
Medway Rainham PCN consists of 9 practices:
